Life Journey Partners of America, Inc is a grassroots movement committed to finding solutions supportive of the millions of Americans who care for individuals living with a chronic illness or disability. Many experience gaps in care or insurance coverage and may need experienced professionals to walk alongside them as they find the help they require. We support family caregivers and those hired to care for our most vulnerable as they strive to be healthy and happy in their work and in their lives

Life Journey Partners of America is a 501 (c) 3 Non-profit Organization whose mission is to empower Caregivers to care for themselves and live their best lives while caring for another. We are committed to building and supporting Caregiving Communities within existing communities to address gaps in care and provide needed resources for Caregivers to access the assistance needed.
We envision a community supportive of caregivers, committed to finding solutions to help them maintain their health while caring for others and remain a part of the community at large through shared services and visits. We have created the Caregiver Support Hub as a template for community centers or churches to follow since they may have the space and vetted volunteers. We want to be part of the solution!
Our Caregiver Support Hub is located at 13225 N Verde River Drive, #101 in Fountain Hills. The space was opened to provide caregivers in the community to have a place where they can come most days of the week to just get away and be with those who understand the caregiver journey. You are invited to join us for a cup of coffee or tea, or just to sit and take a break, knowing you are not alone.

Get DirectionsIn response to the growing strain on caregivers, Life Journey Partners of America, Inc. opened the Caregiver Support Hub aimed at providing education, resources and emotional support for both family and professional caregivers. A Life Journey Partners of America, Inc. project, the Caregiver Support Hub is located at 13225 N. Verde River Drive, Suite 101 in Fountain Hills, Arizona. The mission is simple but vital: empower caregivers to care for themselves while addressing gaps in support for the people they care for.
Dr. Karen Willfahrt, a licensed naturopathic physician and founder of Life Journey Partners of America, Inc. She collaborates with two professionals in the world of Caregiving, Christine Leonor and Crystal Sanchez-Griffiths. The three women hope to provide needed resources and a place that caregivers can feel supported by those who have walked the walk.
Christine Daryll Leonor, a licensed caregiver and owner of Prestige In-Home Care Services, LLC and Crystal Sanchez-Griffiths, CEO and founder of Advocate Partners of Arizona, LLC will collaborate with the nonprofit to facilitate the space.
The Hub features a reception area, a gathering space and a consultation room for private discussions.
Caregivers often spend so much time caring for others that they overlook caring for themselves. According to a 2025 report by AARP and the National Alliance for Caregiving, more than 1.2 million adults in Arizona provide unpaid care to a family member or a friend with complex medical conditions or disabilities.
Nearly 40% of them also work full-time, the report said, placing them at high risk for burnout, stress-related health issues and emotional fatigue. Nationally, an estimated 63 million adults serve as caregivers, underscoring the widespread need for support.
The three facilitators invite caregivers and those they care for into the Hub for coffee, tea and conversation. Staff will help connect visitors with local and regional services, care options, educational programs and emotional support. We hope the Caregiver Support Hub will become a steady source of comfort and guidance for anyone who is caring for others.

Caregivers and Domestic Workers are often key to individuals remaining in the home safely. Those needing care may live with family or friends who work, and assistance is needed for all to live their best lives. Please join our Volunteer Community to answer the call to find solutions for the existing Caregiver Crisis.
Individuals in need of care may be unable to receive the care they need, due to a lack of insurance coverage, inadequate financial resources or staffing issues. Perhaps, with funds available, these issues can be addressed with creative sustainable solutions. Future fundraising opportunities will be available to assist those in need.
The House Share Program may be a solution for the home owner who has extra room, is lonely and may need help around the house. The perfect housemate may be a college student looking for affordable housing or someone who has recently become homeless. We hope to find solutions in the future for those insecure in the home and who are home insecure.
Founder and Director
Dr. Willfahrt is a Naturopathic Doctor and Nurse. She has been working in the healthcare field for over 50 years. She has cared for the most vulnerable of all ages, in the hospital and at home, ultimately caring for her mother in her final years. These experiences inform her current life’s work.
Volunteer Collaborator
Christine is a licensed caregiver and owner of Prestige In-Home Care Services, LLC. She had been a highly trained caregiver when she formed her company to provide individualized caregiving services in 2021. She has advanced training in the various types of dementia.
Volunteer Collaborator
Crystal had been working in the healthcare field for over 20 years when she formed Advocate Partners of Arizona, LLC. She has worked in areas such as Hospice and the various levels of care in the senior communities. Her knowledge of resources available for patients is extensive.
